Modélisation géométrique et mécanique personnalisée de l'appareil locomoteur

Abstract : Most of the models used in gait analysis use generic data, and thereby don't take the patient's morphology (body segment parameters, forces' lever arms) into account. This work consists in enhancing those models, by integrating patient specific geometric and mechanical characteristics. Clinical trials, including a gait analysis, a stereo-radiography with the EOS® low dose biplanar X-ray system, and an MRI exam, are conducted on 10 healthy subjects and 5 patients waiting for an anterior cruciate ligament reconstruction. The bones' and the external envelope's geometry is reconstructed from the stereo-radiographies. Registering the bones into the gait analysis frame enables us to locate precisely the centre of the femoral head (CFH). The patient specific body segment parameters (BSP) are computed and integrated into the model. The impact of using the registered CFH and the BSP can not be neglected, as the difference to the moments computed with the conventional method reaches respectively 2 and 0.5%BW*Ht. Apart from its clinical application, the patient specific muscular geometry is an important input data for muscular models, which determine the forces produced by the muscles, ligaments and the contact areas. We present an original method enabling us to reconstruct the 13 main muscles involved in the knee joint's motion using 6 to 8 MRI images. The reconstruction takes an hour and provides the muscles' volume and surface with an error of 5% and 5mm (2RMS). This project provides the baselines for a patient specific muscular model, which will inform us on the different structures' contribution to the joint forces and moments, and thereby help us to better understand the pathologies and the patients' compensatory strategies.
