Analyse en pré et post opératoire de l'articulation de la hanche à l'aide de reconstructions 3d issues de radiographies biplanaires basse dose

Abstract : Due to ageing, total hip arthroplasty is becoming a common surgery. However complications such as early dislocations may occur. Therefore functional and three-dimensional (3D) evaluation of the hip joint is of major interest for health-care. 3D reconstruction methods were elaborated in vitro, on excised bones, for the proximal femur and the pelvis. The purpose of this research was the in vivo quantitative 3D evaluation of the hip joint. The EOS® device was used to obtain frontal and sagital standing X-rays with extra low dose of radiation. The main difficulty for the in vivo hip 3D reconstruction is the bone superposition on the lateral view. In order to unlock this problem, methods based on the deformation of patient specific models using a database were developed in order to obtain a reproducible and precise reconstruction tool. These methods were developed for the proximal femur and the pelvis. The inter-observer reproducibility was 2.2 mm and 2.6 mm (2 standard deviations) for respectively the proximal femur and the pelvis. These methods were then used for patients needing primary or secondary hip arthroplasty. Standing, sitting and squatting EOS® X-rays were performed. Positional and morphological parameters were then evaluated. Statistically significant differences were found between control subjects and patients, especially for the pelvis position. An in vivo hip analysis tool was developed thanks to this research. The EOS® device potential was also highlighted for 3D quantitative functional analysis of healthy or implanted hip.
