Résumé

The anatomy of the lower limb and its 3D angular abnormalities are paramount in diagnosis, therapeutic applications and/or surgical planning. In this context, it is important to explore this knowledge in three dimensions. Within a collaborative project between the Laboratoire de Biomécanique (Paris) and the Laboratoire de recherche en Imagerie et Orthopédie (Montréal) 3D reconstruction methods have been developed based on the EOS™ imaging system (Biospace Med, Paris). These methods allow clinical analysis in standing position with low radiation dose. However, despite their accuracy, these reconstruction techniques are time-consuming and therefore difficult to use on a routine basis in clinical environment. The aim of this Ph.D. thesis is to adapt and optimize these methods for lower limb's bony structures with structural distortions. We developed and evaluated a reconstruction method of the lower limb based on parametric models of femurs and tibias and statistical inferences. This method provides a preliminary so-called “simplified” model of the lower limb in approximately 1 minute, allowing an accurate and reproducible computation of most currently used clinical parameters. Starting from the “simplified” model, we calculated an accurate customized 3D reconstruction of the lower limb in approximately 5 minutes. Based on this 3D model, we developed and validated several calculation modes for a larger panel of clinical parameters (including torsions and rotations) in order to identify the most robust and pertinent ones that could be standardized for clinical practice. The current work opens practical perspectives for the clinical transfer of such methods to be used in practice. One of the most clinical popular applications of the 3D reconstruction techniques and clinical parameters calculation is the surgical planning of lower limb osteotomy and arthroplasty.
La connaissance précise de l'anatomie du membre inférieur ainsi que les anomalies angulaires suivant les trois plans anatomiques a une importance majeure dans la routine clinique pour le diagnostic, les applications thérapeutiques et/ou pour le planning chirurgical. C'est pour ces raisons qu'il est fondamental d'aborder ces connaissances en trois dimensions. Dans le cadre d'une collaboration franco-québécoise, le Laboratoire de Biomécanique (Paris) et le Laboratoire de recherche en Imagerie et Orthopédie (Montréal) ont développé des méthodes de reconstruction 3D à partir de radiographies biplanes issues du système EOS™ (Biospace Med, Paris). Ces techniques permettent une analyse clinique en position debout et avec de basses doses d'irradiation. Bien que très précises, ces approches présentent le grand désavantage de ne pas permettre un déploiement en routine clinique en raison du temps de reconstruction élevé. L'objectif de cette thèse était donc d'adapter les méthodes de reconstruction 3D pour la modélisation du membre inférieur et le calcul des indices cliniques en présence de déformations structurales. Dans ce but, nous avons proposé et évalué une méthode de reconstruction du membre inférieur s'appuyant sur des modèles paramétrés de fémurs et de tibias et des inférences statistiques. Cette méthode vise à une estimation très rapide (1 minute) d'un premier modèle dit « simplifié » du membre inférieur à partir duquel un groupe d'indices cliniques restreint peut être calculé. A la base du modèle « simplifié », nous avons calculé une reconstruction 3D précise du membre inférieur en un temps relativement réduit (5 minutes). Finalement, et à partir de ce dernier modèle, nous avons exploré et validé différents modes de calcul d'un éventail plus large d'indices cliniques (incluant les torsions et rotations) afin d'en déduire les plus robustes et pertinents qui pourraient être standardisés pour une utilisation en clinique. Ce travail de thèse ouvre des perspectives concrètes en terme d'utilisation de telles méthodes en pratique clinique, en particulier pour la planification des ostéotomies et des chirurgies pour la pose de prothèse.
