Intérêt de la citrulline dans la prise en charge du sujet âgé dénutri : Etudes expérimentale et clinique

Abstract : Protein-energy malnutrition found in the elderly contributes to muscle decline (sarcopenia and dynapenia)and then to an increased morbi-mortality. Prevention and treatment of malnutrition is difficult due to a resistance to renutrition observed in the elderly; this can be explained by a high splanchnic extraction of amino acids at the post-prandial time, which results in a least stimulation of muscle proteosynthesis. Citrulline is an amino acid that escape splanchnic extraction ans is a stimulating factor for muscle protein synthesis. We showed citrulline improves muscle mass and function (motor activity and maximal tetanic isometric force) in malnourished aged rats. This benefic effect could be partly explained by an overexpression of myofibrillar contractile proteins and a decrease in the expresssion of componants of the proteolysis pathway. Its mecanism of action is still unclear but our data suggest citrulline would reorientates energy metabolism. In addition to these experimental data, we develop a clinical study to evaluate the effect of citrulline on protein metabolism in the malnourished elderly. Then, we could evaluate its effect on muscle function in this population. Citrulline could thus represent an efficient and inovative nutritionnal strategy to limit muscle decline that occurs within ageing; it could also limit morbi-mortality due to protein-energy malnutrition.
