Mise en oeuvre d'une tâche virtuelle, écologique et paramétrable dans le contexte des fonctions exécutives

Abstract : Despite the frequency of the impairment of executive functions after brain injury, assessment and rehabilitation approaches of the dysexecutive syndrome are still limited. This alteration is very disabling because it disrupts Activities of Daily Living (ADL) (e.g. meal preparation), prevents return to social and professional life. The technology of virtual reality has shown its potential to propose innovative therapeutic intervention strategies. They require modeling of ecological virtual tasks (close to ADL), follow-up and assessment of the patient's activity in these tasks and finally assessment of the errors performed by the patient. The design of these tasks is based on the traditional practice at rehabilitation units such as Kerpape Rehabilitation Center. The interaction modalities in these tasks should take into account the constraints related to the patients, like their residual abilities. In these contexts, during our thesis work we designed a tool that allows therapists to configure an ecological virtual task for assessment and rehabilitation of patients with brain injury. In the therapeutic virtual kitchen (TVK), which is an accurate representation of the therapeutic kitchen of Kerpape, we designed primary and complex ecological tasks that can be graduated in order to be adapted to patient's abilities and to evaluate his/her executive dysfunctioning. With the purpose to assess the patient's performance in the chosen complex task (Preparation of a coffee), we partially computerized the assessment grid of actions errors and behavioral errors, used in the real task at Kerpape Center. The TVK tool can save in real-time data on patient's activity as well as interpret and record errors. The experiments allowed us to validate the usefulness of the TVK tool with healthy subjects and patients with brain injury, and to provide objectivity in the assessment of the patient's performance within an ADL. Our work brings out the difficulty of assessment of behavioral errors. In the context of brain injury, we proposed a VR-based approach for the assessment of patient's activity while performing an ADL. This approach helps us to highlight some assets of VR in the area of cognitive disorders and some prospects in cognitive rehabilitation.
