Modélisation tridimensionnelle globale du squelette pour l'aide au diagnostic et à la prise en charge thérapeutique des pathologies rachidiennes affectant l'équilibre postural

Abstract : Taking global balance into account may help avoid biomechanical complications when caring some spinal disorders. The EOS™ system (EOS™ imaging, Paris) is designed to investigate the patient head-to-feet specific three-dimensional geometry of the skeleton in clinical routine. This work aims at taking advantage of this new modality to improve caring spinal diseases associated with balance disorders. First, tools, methods and protocols were developed and validated to be able to conduct such an evaluation. Thus, we could quantify postural balance on 93 patients with spinal disorders in clinical routine and compare the calculated parameters with a population of 23 asymptomatic volunteers. This study yielded new criteria to evaluate three-dimensional postural balance in clinical routine. Analyzing postural balance on 25 patients with severe spinal disorders, we could then define a postural model to estimate pelvic tilt after correction of the lumbar curvature using vertebral osteotomies. At last, decision-making support was provided by means of a surgical planning method for these particular interventions. Based on the postural model, this method predicts the postoperative geometry of the skeleton from the preoperative geometric configuration and the intended surgical gestures.
